In its third year of funding, the Symphony Programme (an NHS England Vanguard site) has been awarded £3.76M to continue the development and implementation of innovative new models of primary and acute care. Creating a seamless, integrated healthcare system for the people of South Somerset.

In April 2016, we set up a new primary care organisation, through a partnership between Yeovil District Hospital and local GP practices; which a number of practices have now joined.

By creating new ways of working between primary and acute care, we can offer our patients a more individualised, timely and effective healthcare service.

As our organisation grows, we are now recruiting GPs who want to use and advance their skills, across the south west.
